Admission Forms
APPLICATION, AFFIDAVIT & AGREEMENT FORMS
The following forms are available in Word format or web format. Just select, print, complete in ink and return to International Student Office/Admissions.
- Intent to Transfer Form (for transfer students only)
- Guardian Form (for students under 18 only)
Complete and the International Student applicatrion and the Affidavit of Support. (If tranferring from another U.S. institution, also submit the Intent to Transfer form. For students under 18 who are high school graduates, submit the guardian form as well..) With the forms, submit the following:
* Official transcripts from you high school or college
* an original bank statement
*Two passport-sized photos
*TOEFL or similar standardized English language score
* Non-refundable $35 Fee (bank check or money order.
If from outside U.S., check must be drawn on a U.S.
associate or affiliated bank.))
