You may come in to the Financial Aid Office to pick up these forms or you may download them to your own printer. For 2011-12 some students may not have to submit any forms. Every student must follow the directions they will receive by email beginning in late spring 2011 that will direct them to a website where information must be submitted.

 

About 30% of students are selected for verification. You will be informed if you need to provide a signed verification worksheet and proof of income, usually a signed copy of your and your parents' (if you are dependent) 2010 income tax returns. You may download the Verification Worksheet to submit with the tax returns. If you used the IRS transfer tool when you filed your FAFSA, tax returns do not have to be submitted at this time. You may go to www.fafsa.gov and do a correction to your submitted FAFSA and use the IRS transfer tool to avoid submitting a tax return (if requested). The Verification Worksheet must still be submitted.

 

Verification Worksheet Petition for Extension of Financial Aid 1 Use this appeal form if you have been disqualified because of not completing 67% of your attempted units or maintaining a 2.0 GPA. It may be submitted with an educational plan from a counselor or a quicker solution would be to attend a group counseling session for students who have been disqualified.
